Working Together

For most new clients—especially those with more complex concerns—I recommend starting with a two-appointment approach.

This allows me to spend more time understanding your health history, symptoms, and goals, and to develop a more personalized, in-depth plan.

How to Prepare for Your First Appointment

To make the most of your time, please complete the following in advance:

  • 3-Day Dietary Record
    This helps identify patterns in your current eating habits and areas for improvement. If you have a history of disordered eating, we can discuss your current intake together instead.

  • Healthie Portal Access
    You’ll receive a link to join my HIPAA-compliant client portal.

  • Welcome - New Client Information Form
    please review this information within the Healthie portal prior to your appointment

  • Intake Form
    Please complete your intake form within the Healthie portal prior to your appointment.

  • Office Policies and Informed Consent Form
    please review this information within the Healthie portal prior to your appointment

  • Recent Lab Work (if available)
    Upload lab results from the past 3–6 months to Healthie or bring copies to your appointment. If you don’t have recent labs, we can discuss next steps.
